Is Diabetes Hereditary? The Actual Cause

It has become a common excuse for most medical professionals to put the blame on genetic heritage or the DNA for the diseases that they cannot find the cure or solution for. It is like the ultimate escape for them when they are forced by a patient's family for a reasonable answer to the patient's complicated health problem. It will further convince their assumption when they are told that someone among the family members of the patient is having the related disease too.

You can see this phenomenon happens in almost all modern diseases, be it heart disease, diabetes, cancer, etc. Take hereditary diabetes as an example, it is almost certain that a patient's parents or one of his close relatives is having diabetes too. Most of the time, the doctor will tell the patient that his problem is genetically related and there is no cure for his illness. All they can do is to prescribe some drugs to control of maintain his condition.

Maintain? Control? The ironic facts and figures tell us that those expensive drugs failed to deliver what they are meant for. Not only the condition is not getting any better or controlled, as a matter of fact, it gets worse! You don't need any official studies or reports to tell you that, for it is hard for you to find any. Well, that is just part of the drama of the medical money making industry.

It is not the genetic hereditary between a father (or mother) and son that gives both of them the same diabetes problem. My be genetic similarity does contribute a bit, but mostly it is caused by what I called "Diet Hereditary". Well, the odds are high for the children to follow their parents' diet as they grow. If someone is born in a carnivorous family, chances are high he or she will favor meat or fast food like the rest of the family.

There is a popular saying goes like this... You are what you eat. But I would like to add in another line for that quote. What you eat is what your family eats. Food is the actual cause of hereditary diabetes. Everything is so common sense, but common sense is not that common after all. We are trapped inside a modern society where we tend to believe in something more complex or high tech instead of believing in simple common sense.

It is sad but true. Nowadays people surrender their health to the hands of the medical industry. Most of them dare not make any health related decision for themselves or their love ones. They just node their heads for whatever diagnosis or recommendation told by the medical professionals. And even this habit itself is hereditary. Make sense?

Insulin resistance is a condition in which cells, particularly those of muscle, fat, and liver tissue, display “resistance” to insulin by failing to take up and utilize glucose for energy and metabolism (insulin normally promotes take up and utilization of blood glucose from the blood stream).

In its early stages, the condition is asymptomatic, but may develop into Type II Diabetes. Although there are several established risk factors, the underlying cause is unknown.

It has been estimated that 30 to 33 million Americans are insulin resistant, and the number appears to be increasing.

Insulin resistance is initially asymptomatic, and in its early stages can be detected only by laboratory tests. These tests will show an abnormally high blood sugar (glucose) level, but not high enough to be considered prediabetic or diabetic.

While the condition does not always lead to further problems, the majority of people who reach the pre-diabetic level go on to develop Type II Diabetes (formerly called Maturity Onset Diabetes.

Causes and symptoms

The cause of insulin resistance is unknown, although the condition has been seen to run in families, indicating that there is a genetic association. Being overweight, and lack of exercise are also associated with insulin resistance, although the nature of the relationship is not clear. Risk factors for insulin resistance are:
  • having a family history of diabetes
  • having a low HDL (good) cholesterol level—and high serum lipids
  • having high blood pressure
  • having a history of diabetes during pregnancy, or having given birth to a baby weighing more than 9 pounds
  • being a member of one of the racial groups that appear to have a high incidence of insulin resistance (African American, Native American, Hispanic American/Latino, or Asian American/Pacific Islander)
  • having syndrome X
  • being obese

In its mildest form, insulin resistance causes no symptoms, and is only recognizable on laboratory tests. In more severe cases, there may be dark patches on the back of the neck or even a dark ring around the neck. The dark patches are called Acanthosis nigricans and may also cause darkening of skin color in the elbows, knees, knuckles, and armpits.

There is a constellation of symptoms now called metabolic syndrome or insulin resistance syndrome that is linked to insulin resistance. This syndrome was formerly called syndrome X. Metabolic syndrome is defined by the National Cholesterol Education Program as the presence of any three of the following conditions:
  • excess weight around the waistline (waist measurement of more than 40 inches for men and more than 35 inches for women)
  • high levels of serum triglycerides (150 mg/dL or higher)
  • low levels of HDL, or “good,” cholesterol (below 40 mg/dL for men and below 50 mg/dL for women)
  • high blood pressure (130/85 mm Hg or higher)
  • high fasting blood glucose levels (110 mg/dL or higher)

Note that the numbers are those from an expert panel convened by the National Institutes of Health in 2001. Other panels of similarly qualified experts have given slightly different definitions.

Diagnosis

The only means of diagnosis for insulin resistance is laboratory tests. While there are several tests that may be performed, the two most common screening tests are the fasting blood sugar test and glucose tolerance test.

Fasting blood sugar measures the blood glucose level after a 12-hour fast (no food). A normal level, according to the United Sates National Institute of Diabetes and Digestive and Kidney Disease (NIDDK), should be below 100 mg/dL (milligrams of glucose in every deciliter of blood. A value in the n the 100 to 125 mg/dL range is considered evidence of insulin resistance, and is considered prediabetic.

A value of 126 mg/dL is considered diabetic. (Blood sugar levels after a 12 hr fast are typically lower than this, and are controlled by pancreatic insulin secretion that transports blood glucose out of the blood and into the muscles, brain, organs, and other tissues.)

The glucose tolerance test is performed after the patient has had nothing but water for 10 to 16 hours. The patient has his blood drawn for a a baseline blood glucose level. Next, the patient drinks a special sweetened test drink that contains exactly 75 grams of glucose (pregnant women are normally given 100 grams of glucose.)

Blood is drawn again at one-half hour and each of the next six hours to compare blood glucose levels and watch their pattern in response to the sweet drink. Normally the blood sugar levels is lower before the drink, rises quickly during the first few hours, and slowly drops again.

In insulin resistance, the blood sugar level rises but stays abnormally high because it is resistant to being removed from blood into tissues by insulin. High blood sugar from food or the test glucose drink stimulates the pancreas to secrete insulin into the blood.

However, in insulin resistance, the insulin is secreted but is only partially absorbed by the tissues. According to the National Diabetes Information Clearinghouse (NDIC) a normal level would be below 140 mg/dL 2 hours after the drink.

If it is in the 140 to 199 mg/dL range 2 hours after drinking the solution, the diagnosis is impaired glucose tolerance (IGT) or prediabetes. A level of 200 or higher, if confirmed, represents a diagnosis of diabetes.

Type 1 Diabetes Death Rate is Falling, But Not Fast Enough

Average rate is still 7 times higher with the disease vs those without it,- study finds

Death rates have dropped significantly in people with type 1 diabetes, according to a new study.

Researchers also found that people diagnosed in the late 1970s have an even lower mortality rate compared with those diagnosed in the 1960s.

"The encouraging thing is that, given good [diabetes] control, you can have a near-normal life expectancy," said the study's senior author, Dr. Trevor J. Orchard, a professor of epidemiology, medicine and pediatrics in the Graduate School of Public Health at the University of Pittsburgh, Penn.

But, the research also found that mortality rates for people with type 1 still remain significantly higher than for the general population -- seven times higher, in fact. And some groups, such as women, continue to have disproportionately higher mortality rates: women with type 1 diabetes are 13 times more likely to die than are their female counterparts without the disease.

Results of the study are published in the December issue of Diabetes Care.

Type 1 diabetes is an autoimmune disease that causes the body's immune system to mistakenly attack the body's insulin-producing cells. As a result, people with type 1 diabetes make little or no insulin, and must rely on lifelong insulin replacement either through injections or tiny catheter attached to an insulin pump. Insulin is a hormone that allows the body to use blood sugar.

Insulin replacement therapy isn't as effective as naturally-produced insulin, however. People with type 1diabetes often have blood sugar levels that are too high or too low, because it's difficult to predict exactly how much insulin you'll need. When blood sugar levels are too high due to too little insulin, it causes damage that can lead to long term complications, such as an increased risk of kidney failure and heart disease. On the other hand, if you have too much insulin, blood sugar levels can drop dangerously low, potentially leading to coma or death.
These factors are why type 1diabetes has long been associated with a significantly increased risk of death, and a shortened life expectancy.

However, numerous improvements have been made in type 1 diabetes management during the past 30 years, including the advent of blood glucose monitors, insulin pumps, newer insulin, better medications to prevent complications and most recently continuous glucose monitors.

To assess whether or not these advances have had any effect on life expectancy, Orchard, along with his student, Aaron Secrest, and their colleagues, reviewed data from a type 1 diabetes registry from Allegheny County, Pennsylvania. The registry contained information on almost 1,100 people under the age of 18 at the time they were diagnosed with type 1 diabetes.

The children were sorted into three groups based on the year of their diagnosis: 1965 to 1969, 1970 to 1974 and 1975 to 1979. As of January 2008, 279 of the study participants had died a death rate that is 7 times higher than would be expected in the general population.When the researchers broke the mortality rate down by the time of diagnosis, they found that those diagnosed later had a much improved mortality rate. The group diagnosed in the 1960s had a 9.3 times higher mortality rate than the general population, while the early 1970s group had a 7.5 times higher mortality than the general population. For the late 1970s group, mortality had dropped to 5.6 times higher than the general population.

The mortality rate in women with type 1diabetes remained significantly higher, however, at 13 times the rate expected in women in the general population.

In addition, blacks with diabetes had a significantly lower 30-year survival rate than their white counterparts -- 57 percent versus 83 percent, according to the study.

Although Orchard said it isn't clear why women and blacks have higher-than-expected mortality, Barbara Araneo, director of complications therapies at the Juvenile Diabetes Research Foundation, said that both discrepancies have been found in other research, and that one theory is that blacks may have a greater genetic susceptibility to heart disease or high blood pressure. And, for women, she said previous research has shown that, "women with diabetes lose their innate protection against [heart disease], similar to the loss sustained in postmenopausal phases of life." But, she said, it's not clear how diabetes causes this loss.

The overall message of the study, however, is a positive one.

"The outcome of this study shows that diabetes care has improved in many ways over the last couple of decades, and as a result people with diabetes are living longer now," said Araneo, adding, "Managing and taking good care of your diabetes is the surest way to reduce the risk of developing complications later in life."

"What we're seeing now is incredibly encouraging, but it's not necessarily the full story yet," said Orchard, who noted that improvements in diabetes care should continue to lower mortality rates in people with type 1 diabetes.

Hypoglycemia without diabetes

Low blood sugar, or hypoglycemia, is common in diabetics with type 1 diabetes and occurs occasionally in people with type 2 diabetes who are taking oral hypoglycemic drugs. Hypoglycemia is the result of a mistake in insulin dose, exercise, and available carbohydrates usually because of a missed meal.

The symptoms of hypoglycemia result from a compensatory autonomic nervous system response and from impaired cerebral function due to a decrease in glucose available for use by the brain. The onset of hypoglycemia in diabetics is sudden with blood glucose levels of less then 45 to 60 mg/dL. In severe cases death can occur.

For the diabetic there are two forms of hypoglycemia that they can experience; mild and severe.

For mild hypoglycemia immediate treatment is necessary. Diabetics experiencing hypoglycemia should take about 15g of a rapid-acting sugar. This amount of sugar is found, for example, in three glucose tablets, ½ cup of fruit juice or regular soda, 8 oz of skim milk, five Life Savers candies, three large marshmallows, or 3 tsp of sugar or honey. Sugar should not be added to fruit juice. Adding sugar to the fruit sugar already in the juice could cause a rapid rise in blood glucose, causing persistent hyperglycemia.

If the symptoms of hypoglycemia continue the 15/15 rule should be followed. Wait for 15 minutes, check blood glucose levels, and if it is still low eat another 15 grams of carbohydrate. This procedure can be repeated until blood glucose levels return to normal. Diabetics should have a source of carbohydrate readily available all the time so that they can quickly deal with hypoglycemic symptoms. If the hypoglycemia continues to happen more than two or three times a weeks an adjustment of the diabetes management plan may be in order.

Diabetics who experience severe hypoglycemia often need to be hospitalized to get their symptoms under control. The criteria for hospitalization are one or more of the following.

• Blood glucose is less than 50 mg/dL, and the prompt treatment of hypoglycemia has not resulted in recovery of symptoms.
• The diabetic has coma, seizures, or altered behavior.
• The hypoglycemia has been treated, but a responsible adult cannot be with the client for the following 12 hours.
• The hypoglycemia was caused by a sulfonylurea drug.

If the diabetic is conscious and alert they should be given 10 to 15 g of an oral carbohydrate. If they are semi-conscious glucose or glucagon will be given by injection or IV which is the fastest way to increase blood glucose levels. Glucagon is used in severe cases of insulin induced hypoglycemia to rapidly raise glucose levels. It acts fast but does not last long so it is important that some form of carbohydrate be administered after its use to prevent a recurrence of hypoglycemia. In cases of unconsciousness glucagon may cause vomiting when he diabetic regains consciousness.

Diabetic hypoglycemia is not a condition to be taken lightly. At the first signs of its effects it is important to start taking action to counter its effects.
